Initial Assessment
We begin with an in-depth consultation to understand the unique needs of your loved one. This includes medical requirements, lifestyle preferences, budget, and location preferences. By gathering all necessary information, we ensure that every recommendation is tailored to your loved one's specific needs.
Options Presentation
After the assessment, we curate a list of suitable home placement options, such as independent living, assisted living, or memory care, based on the individual’s needs. We provide detailed information on each option, including services offered, costs, and amenities, so families and case managers can compare and evaluate the best fit without feeling overwhelmed.
Facility Tours & Community Fit Assessment.
To further simplify the process, we arrange and accompany families or case managers on facility tours. During these visits, they can meet staff, See and engage with the community firsthand, and ask any questions. We handle all the logistics, making sure everything runs smoothly and efficiently.
Decision Support
Once the tours are complete, we offer ongoing support to help navigate the decision-making process. Whether it’s answering additional questions, providing guidance on comparing communities, or assisting with paperwork, we are there to ensure families feel confident in their choice.
Ongoing Support
Even after placement, our services don’t stop. We offer ongoing support to ensure the transition is smooth and to address any concerns that may arise. If needs change, we are available to assist in finding new solutions.
